会員登録をされる方退会される方は下記の申請書と制度について
ご確認のうえ、申請をお願いいたします。
A各種申請書（退会届）の原本を、総本部事務局までご送付ください。
各種申請書 (Various Applications Form)
Please send the Various Application Form to the Sohonbu Office. For withdrawal from the dojo (monthly fee), please directly contact the dojo.
所属道場の退会とは別に、総本部会員登録へ退会申請をお願いいたします。
各種申請書（退会届）に必要事項をご記入のうえ原本を、総本部事務局までご送付ください。
なお、振替日の前々月末日（例：5/8振替→3月末）までに受理できなかった場合、会員登録は自動更新され年会費のお支払義務が発生しますので、ご了承ください。
上記手続きが完了済みにもかかわらず、はがきが届いた場合には総本部事務局までご連絡ください。
In addition to withdrawing from the dojo, please apply for a withdrawal from the Sohonbu membership system. Please fill out the necessary information in the Various Application Form, and send it to the Sohonbu Office. Please confirm that if we cannot receive the application of withdrawal by the end of the month, 2 months before your payment month (ex. transfer on May 8 →submit form by the end of March), the registration will be automatically renewed and you will be required to pay the annual fee. If you have received a postcard from the Sohonbu Office even when the above withdrawal process has been completed, please contact the Sohonbu Office.
引き落とし日の前々月の末日までに退会手続きをしていただきましたら、次年度からお引き落としはされません。
退会手続きをしていただくまで会員登録は自動更新され、年会費のお支払義務が発生しますのでご了承下さい。
If you can complete the withdrawal process by the end of the month, of 2 months before your payment month, the annual fee will not be transferred from the next year. Please confirm that the registration will be automatically renewed until the withdrawal process is completed, and until then, you will be required to pay the annual fee.
会員マイページ内の「会員情報」に記載されている振替日が引き落とし日となります。
引き落とし日が土日祝日の場合、翌日引き落としとなります。
※家族会員様は記載日と異なる場合がございますので、総本部事務局までお問い合わせください。
The date written on the “Member Information” in the Membership Page is the timing of the payment for each year. If the date of transfer is on a Saturday, Sunday, or a national holiday, the transfer will be done on the following business day. *For family members, the timing of the transfer may not be the date written on the card, so please contact the Sohonbu Office for details.
2023年5月以降、会員証の発行廃止に伴い会員証の再発行は行っておりません。
会員情報は会員マイページに集約されております。
今後は、会員マイページをWEB会員証としてご利用ください。
The issue of membership cards ended in April 2023. The member information can be viewed from the Member My Page. Please use the website as an online membership ID.
翌月の8日に再度お引き落としさせていただきますので、引き落とし日の前日までにご指定口座へご入金をお願いします。
The payment will be transferred again on the 8th of the following month, so please prepare the necessary amount in your account by the day before the transfer.
令和元年12月9日引き落としから11,000円です。一部のスポーツクラブは3,300円です。
From the transfer on December 9, 2019 the annual fee is 11,000 yen for regular members, and 3,300 yen for sports club members.

ご申請頂いた口座に不備があったため、お送りさせていただきました。
今年度分の年会費は払込取扱票でご入金下さい。

Please make the payment for this year’s annual fee by utilizing the postal transfer document. For the transfer of the annual fee from next year, please fill out the membership application form again.

下記の手順に従って、会員マイページの登録写真の変更を行ってください。
【申請手順】
① 所定の大きさ（縦4cm×横3cm）の写真を1枚ご用意ください。
② 写真の裏面に「会員番号」「会員氏名」「支部名」を必ずご記入ください。
※ご記入がない場合、正しく変更されない可能性がございます。
③ 各種申請書「その他」欄に会員証の写真変更希望であることをご記入ください。
④ 写真と各種申請書を1セットにして道場の先生にお渡しください。
Due to the end of the issue of Membership Cards, we do not reissue cards including change of photos.
Please change photos in the following directions.
① Take a photo in H 4cm×W 3cm size.
② Write your “Membership Number”, ” Member Name”, and “Branch Name” on the back of the photo
*The change may not proceed properly if the information is not written
③ Write “Change of photo” in box no. 6 of the Various Application Form
④ Hand in the photo and the filled Various Application Form to the dojo staff

もしくは、ホームページよりダウンロードをしていただくか、FAX、郵便でお送りさせて頂きます。
You will be able to receive the form at your dojo. You can also download the document from our website, or if necessary, we can send the document to you by FAX or mail.

稽古中のケガについては新極真会独自の傷害共済制度が適用されます。
本制度を申請される場合は必ずケガをした当日に支部長・道場長または指導員に申し出てください。
詳しくは各支部道場、または総本部事務局会員サービス課まで。
For injuries during training, the accident insurance system for Shinkyokushinkai will be applied.
To apply for this system, please make sure to discuss the matter with the Branch Chief/Contact on the day of the injury. Please contact the Branch Chief/Contact or the Sohonbu Office for details.
